In general the cardiometabolic team will counsel to each patient with the recommendations in order to achieve an optimal prevention strategy in order to reach a correct guidelines recommended ldl-c therapeutic regimen.

- The project aims to set up, as recommended by available guidelines, a multidisciplinary cardiometabolic team to make decisions for the management of patients with diabetes mellitus (dm), chronic kidney disease (ckd) and peripheral artery disease (pad).
